Once your first session is scheduled, please read the Counseling Services Contract below and make note of any questions to address with David.
An electronic version of this document will be emailed
for your signature prior to the first session.
Using Insurance

Your mental health benefits may be different from your medical benefits.
David recommends you contact your insurance company by calling the number on your card and ask the following questions:
Is David Phelan, LPC a provider in your network?
What is the copay or coinsurance for each session?
Is there a deductible for mental health benefits? If so, how much and how much has already been met?
Is there a limit to the number of outpatient psychotherapy visits?
If you would like our office to submit claims to your insurance for you, a copy of the front and back of your insurance card, and a copy of your (or the insured’s) photo ID is required prior to the first session.
You may send it by email, US Mail or fax.
